Jacqueline M. Ferebee Obituary, Death – A Cleveland woman driving a three-wheeled vehicle was killed early Monday morning after crashing into a closed gate at Mentor Headlands Beach State Park, according to the Ohio State Highway Patrol (OSHP). The accident occurred just before sunrise, with OSHP deputies responding to the scene at approximately 5:08 a.m. on September 17. The crash took place at the entrance to Headlands Beach State Park, located just north of State Route 44 (SR-44).

Deputies determined that the vehicle involved was a 2021 Polaris Slingshot, which was traveling northbound on SR-44. As it approached the park entrance, the driver struck the gate that was positioned across the roadway because the park was still closed.

Following the collision with the gate, the Polaris Slingshot continued northbound, veering off the road and coming to a stop in the brush on the east side of the road.

The driver of the vehicle has been identified as 42-year-old Jacqueline M. Ferebee from Cleveland. She was the sole occupant of the Slingshot and was reported to have been wearing her seatbelt at the time of the accident.

The OSHP has not yet determined whether factors such as alcohol, drugs, or speed contributed to the fatal crash. The investigation into the circumstances surrounding the accident is ongoing.
